New patient therapy appointments are not guaranteed and may be placed on a waitlist. The provider will confirm whether their schedule allows for therapy appointments.
Do you provide disability or school/work accommodations?
Yes, we do provide disability and/or accommodation paperwork for a fee of $100. The decision to complete these forms are determined after a series of sessions. What if I need a Prior Authorization for my medication?
Once we receive notification from your pharmacy that a Prior Authorization is required, we will submit the necessary paperwork to your insurance company. Approval of the Prior Authorization from your insurance company may take up to 2 weeks.
Why was my intake appointment cancelled?
An intake appointment is usually cancelled due to the forms not being completed or payment and insurance information not being valid. It is very important that all forms and required documents are provided to ensure that your appointment is confirmed.
